CNIC / Form B five attested copies (original not required). Board Migration Certificate (NOC) in case of F.Sc is federal board or other than Punjab along with five attested copies. Certificate that you have been vaccinated against Tetanus, Measles, Mumps & Rubella for (Girls). Certificate that you have had a full course of immunization against Hepatitis 'B', 'C' & Corona Virus.
